The aim of this study is to investigate whether the consumption of a new pharmaceutical product containing the active ingredient sildenafil can reduce the incidence of the adverse effects of the newly developed PDE-5 inhibitor (Viagra) or to determine the effect of a new treatment to be administered to the patient on sexual function and overall satisfaction with the use of the drug. A series of randomised, open-label, clinical trials were conducted in a parallel population of patients in a tertiary university teaching hospital in southern Thailand, in order to investigate the efficacy and safety of the new product sildenafil, and the effects of sildenafil treatment on sexual function, orgasmic function, sexual satisfaction, and self-esteem in the long term. A total of 10,945 men (average age 64 years) were randomised to treatment with the drug sildenafil 20 mg, 100 mg, and 200 mg, in which the use of sildenafil was preceded by sexual function and self-esteem. The mean age was 63.3 (range 23-83) years; the male to female ratio was 1:1.5. The total number of sexual partners was 4.3%. The mean number of partners was 1.8 (range 1.4-4.9). Of the 10,945 men, 5.1% were female and 1.6% were male. The mean duration of the erections was 17.4 (range 17-22) minutes; the mean duration of orgasm was 2.1 (range 1-5) minutes. The mean total number of partners was 8.6 (range 4-11). Of the 11,882 men, 4.8% were female and 2.8% were male. The mean duration of the orgasms was 1.9 (range 1.3-2) minutes; the duration of the orgasm was 2.7 (range 1.1-4) minutes. The mean overall satisfaction with the use of the drug was 84.1 (range 66-92) out of 10,945 men. In the long term, sildenafil treatment for erectile dysfunction is associated with a lower frequency of adverse effects than the placebo, but no significant effects on sexual function, orgasmic function, or self-esteem in the long term.
